What is a Letter of Medical Necessity?

A Letter of Medical Necessity (LMN) is a document from a licensed healthcare provider stating that a product is medically required to treat or manage a specific condition. The IRS requires this documentation for dual-use items like mattresses to qualify as eligible HSA or FSA expenses.

Flex facilitates this through a telehealth consultation at checkout. You do not need a separate doctor appointment or a note from your chiropractor in advance. The process completes entirely during checkout and typically takes about 2 minutes. Your LMN is accessible at any time at withflex.com/portal.

Keep your LMN and your Spinal Rest receipt together for at least 3 years. Your FSA administrator may request the LMN to confirm eligibility.

Spinal Rest has partnered with Flex to allow you to use your HSA or FSA funds at checkout. Flex facilitates a brief telehealth consultation that determines your eligibility for a Letter of Medical Necessity. If approved, your purchase can be processed using your HSA or FSA card, or you can pay with a credit card and submit for reimbursement. Flex handles the entire qualification process. Spinal Rest does not review your health information directly.

You can still go through the Flex checkout process using a regular credit card. After purchase, log in to withflex.com/portal using the same email you used at checkout to download your Letter of Medical Necessity and itemized receipt. Submit those documents to your HSA or FSA administrator for reimbursement.
The most common reason for a declined HSA or FSA card is insufficient funds. Contact your HSA or FSA administrator to confirm your available balance. If the decline is for another reason, email us at sleep@spinalrest.com and we will work with Flex to help resolve it.

A Letter of Medical Necessity (LMN) is a document from a licensed healthcare provider confirming that your purchase addresses a specific medical condition. It reclassifies a mattress or sleep product from a general consumer item to a qualifying medical expense. Keep your LMN on file with your Spinal Rest receipt for at least three years in case of an audit of your HSA or FSA account.

Using multiple HSA or FSA cards in a single transaction is not currently supported. If your account balance is lower than the purchase total, you can enter a credit card at the Flex checkout and then submit your LMN and itemized receipt to your administrator for reimbursement of the eligible portion.
Yes. Sales tax on eligible items is covered by HSA and FSA funds. If your cart contains both eligible and non-eligible items, the tax is divided proportionally.
Most FSA administrators require purchases to be made and submitted during the plan year in which your FSA coverage is active. HSA rules are more flexible and generally allow reimbursement at any time, even years after purchase. Check your specific plan’s rules to confirm.
State restriction: Due to state telehealth laws, Flex is unable to issue Letters of Medical Necessity to customers in Louisiana, New Mexico, and Mississippi. Customers in these states cannot use the Flex HSA/FSA pathway. They may still purchase using a standard payment method and seek reimbursement through their own licensed healthcare provider independently.
